Infectious mononucleosis (also known as "mono" or the "kissing disease") is a fairly common viral disease that usually affects adolescents and young adults, including college students.

Immunization records form (online):
All new students (or their parent/guardian) should submit their immunization records, preferably at least 30 days before classes start. This information may be used to provide health care for students.
Parent / Guardian Consent for a Minor to Receive Health Care Services:
Only for students under the age of 18; a parent or guardian should complete this form.

Medications for HIV Prevention
Two medications are approved to help prevent human immunodeficiency virus (HIV) infection. Both medications are prescribed at UHS.

Mental Health Services
UHS offers a variety of helpful mental health services, including evaluation and treatment, medication management, specialty clinics, Clinical Social Workers, and the Addiction Medicine Clinic. Wolverine Wellness provides non-clinical services such as Wellness Coaching, and the Collegiate Recovery Program.
Primary Care Clinic
In the Primary Care Clinic, you will receive care in one of three different areas on the first floor of UHS: Medical Clinic, Treatment Center, or Quick Clinic.
